Please complete and submit the following forms if your child has not been screened by Parents As Teachers using the DIAL screener at Taylor Early Childhood Center or his/her preschool this school year.  PLEASE ONLY SUBMIT ONE FORM PER STUDENT. Appointments will last approximately an hour and a half and are filled on a first come first serve basis.

There will be a link at the end of this form to select a date and time for your student’s appointment.

Dunklin R-5 Kindergarten Enrollment Screenings will be held by appointment only on Monday, March 1st, 2021 and Tuesday, March 2nd, 2021 at Zion Lutheran Church, 310 Central Ave., Pevely, MO 63070.  This event is for all children who will turn five BEFORE August 1, 2021.

Due to social distancing and space limitations we are requesting only one guardian and the child being screened to attend.  All registration activities will be socially distanced and adults will be required to wear a mask.  Masks are encouraged for students being screened.
