Please complete this form for each child at the academy, we require your consent to use pictures, video and audio recordings on our website, social media channels and with newspapers and news outlets.

You can change your child's consents at any time by contacting the office on 0121 675 3985 or by completing this form.

Please allow 24hrs for changes to be applied to your child's records.
