This is a Release of Student Data Notice to Parents for school year 2023-2024. This form is to be completed only by those parents who DO NOT wish to share information with St Paul Public Library (SPPL) to get a free virtual library card for their students at CVA. If you wish to OPT OUT of this program, please read and complete the following sections by September 1, 2023.

Note: If CVA does not receive this form from parents by the due date, students are included in the SPPL Library Go program by default.
