Each day that you wish to participate in a school sponsored activity you will first need to fill out this survey. No one will be allowed to participate in any meetings or workouts without first completing this survey. It is your responsibility to answer each of these questions honestly to ensure everyone's health and safety. Upon arrival you may be asked to have your temperature taken and recorded for safety reasons.
