I am the parent or legal guardian of ____________________________ (student). The Pine City Ambassador Junior Leadership Program, its agents and its employees have my full permission and consent to transport and otherwise provide transportation for my child by public service bus, private automobile, vans or other appropriate means of transportation in connection with any session of the Pine City Ambassador program during the term in which he or she is a participant in the Pine City Ambassador Program. I hereby hold the Jackson Area Chamber of Commerce, board of directors, staff, and volunteers, harmless of any accident, injury, illness or any damage whatsoever related to the above mentioned student’s attendance at or participation in any session of the Pine City Ambassador Program. Periodically, students participating in the Pine City Ambassador Program events may be photographed, filmed, or interviewed. As the parent and/or legal guardian of ___________________________________ (student), I grant permission for my child to participate in photograph, films, and interviews as they pertain to the Pine City Ambassador Program and I understand that such pictures, film or interviews may be used to promote or publicize the Pine City Ambassador event or demonstrate the effectiveness of the program. *
