Application for Determination of Residence of a Minor Living Apart From the Parent, Guardian or Other Person Having Lawful Control under Court Order.  This application must be submitted on behalf of each person under 18 (eighteen) years of age who claims a residence in the Channelview Independent School District separate and apart from the residence of the Student's parent, guardian or other person having lawful control of the student under an order of a court (hereafter called “parent, guardian or court custodian”, whether there are one or more):

False Information

When accepting a child for enrollment, the District shall inform the person enrolling the child that presenting false information or false records for identification, is a criminal offense under Penal Code 37.10 and that enrolling the child under false documents makes the person liable for tuition or other costs as provided below.  Education Code 25.002 (d).

A person who knowingly falsifies information on a form required for a student’s enrollment in the District is liable to the District, if the student is not eligible for enrollment, but is enrolled on the basis of false information.  For the period during which the ineligible student is enrolled, the person is liable for the maximum tuition fee the District may charge (see FDA) or the amount the District has budgeted per student as maintenance and operating expense, whichever is greater.  Education Code 25.001 (h).

Change in the residential status of the person (s) under my guardianship will be reported immediately to the Assistant Superintendent of the Channelview Independent School District at 281.452.8002.


Responsibilities of Parent Assumption

• Provide for physical need of child
• Insure required daily attendance of child or pay financial cost incurred for excessive absences
• Teach child to comply with rules and participate in all disciplinary actions for non- compliance with student code of conduct
• Insure attendance at required tutorials and assist with study at home
• Participate in school conferences to discuss child’s progress and educational programming
• Control child or pay for property damaged caused by child
• Maintain up-to-date telephone numbers at school for home, work and emergency
