CPFTA Application 2021
Welcome to Chicago Police and Firefighter Training Academy Application Site.

CPFTA Class of 2023 is now accepting applications.  The deadline for submitting is Monday, May 3, 2021.

Application Requirements
• A resident of the city of Chicago
• You are a current HS sophomore
• 2.3-grade point average by the end of Sophomore year
• Good attendance (80% and above)
• Complete an online application
• Complete a Program Consent Form
• Hold a valid state ID or driver’s license at the time classes begin
• Able to present proof of a current physical examination in September of 2021
• Complete a background check that includes fingerprinting and drug test

Application Process

Part 1 - Complete the Online Application
All applicants have until Monday, May 3, 2021, to complete the online portion of the CPFTA application.   As part of the online application, applicants are required to share the completed Program Consent Form. The consent form must be sent to CPFTA@cps.edu as a PDF, JPEG or PNG.
                                                                       
After CPFTA receives your completed Program Consent Form your online application will be complete.  Candidates will be notified via email if you are eligible to move to part 2 of the CPFTA Application.

Part 2- Background Test
Once part 1 of a student’s CPFTA online application has been accepted, he/she will be notified to report to the Chicago Police Department Education & Training Academy to complete part 2 of the CPFTA application.  

Upon arrival at the Training Academy, students must turn in the following information to the CPFTA staff.  The forms include
1. Personal History Questionnaire (will be sent to students via email)
2. Background investigation form (will be sent to students via email)
3. Authorization for Release of Personal Information form (will be sent to students via email)
4. 2 Letters of Recommendation (one from a school official and one from a community leader)
5. 2 Copies of student’s Driver’s License or State Identification Card
6. Copy of High School Transcript for G.P.A. and Attendance verification

Students will be notified 3-4 weeks after processing regarding acceptance into the program.
