Parents/Guardians: Please have your child complete and submit this form by Wednesday June 16, 2022. Students who do not submit this form will be randomly assigned a semester elective. Electives are courses in addition to the required grade level content area courses.

Students who qualify for intensive courses will be placed in an intensive Reading or Math class in lieu of an elective. We will try our best to put students into their 1st and 2nd choice but an alternative placement may be necessary.

Please pick 3 elective preferences. There is NO guarantee that the student will receive the courses they prefer.

Space in each elective is limited. Scheduling of a student’s academic class requirements will sometimes not allow the selected elective.
