This consent form is for participation in tests designed to detect asymptomatic coronavirus cases. We have been asked by the Department for Education to supervise these tests at the start of the school year again. We would strongly suggest that all students take these tests before our community starts mixing more widely.

Please make sure you have read the Privacy notice before giving your consent for your child to take LFTs.

We only need consent for new students to the Academy. You do not need to complete a form for students who already attend the Academy and who took part in the testing last year. If you have more than one child joining us in year 7, or across different year groups you will need to do a separate form for each child (Please do not fill in a consent form for students joining us in year 12, as they are asked to complete their own consent forms).

If you do not wish to give consent for your child to take LFTs onsite, you can tell us that using this form too.
