Northlake Library ~ Curbside Book Request Form
It is important for your student to keep reading. Curbside service will be provided on Tuesdays and Thursdays.  

Please fill out this form to request books. Request forms need to be turned in at least 24 hours in advance of your selected pickup day. Call 972-494-8359 to let me (Mrs. Lee) know that you are outside in the front of the school. I will bring your materials to your car. Please remain in your vehicle. You only need to return materials if you are finished with them. There are no due dates. You may return books when the next book(s) is delivered.
