If you have any of the below symptoms or if you have been in contact with a suspected or confirmed case of Covid-19, please DO NOT LEAVE YOUR HOME OR ATTEMPT TO ENTER THE STADIUM.

Please Note: If you are the lead booker you will be filling in the below form for your whole party, this includes yourself and any guests you choose to bring.

The answers to this questionnaire will be crosschecked with our access control system. If you do not complete this form on the day your ticket will be deactivated.
