I state that I am a Participant at a free public educational event hosted by Epona Therapy Services, LLC.  I understand that this educational event involves activities using goats and nature based activities which have an inherent risk.   As the undersigned, I hereby forever release, discharge, and hold free and harmless Epona Therapy Services, LLC and all faculty, staff, directors, officers, agents, employees, interns, volunteer personnel, and successors and assigns, for any and all claims, demands, or damages of any and every kind and nature which I, the undersigned, may have now or in the future against the above named organization or persons. This release includes but is not limited to claims of wrongful death or personal injury to me, the undersigned.

I have read and I understand the above paragraphs. I understand that NO LIABILITY can be accepted by any of the above businesses or persons associated with this educational event in the event of any accident or problem concerning me or my children.

I consent to authorize Epona Therapy Services, LLC to take photographs or audio-visual materials taken of myself, children or family.  The photographs and/or audio-visual materials can be shared with me, used for promotional material, or education activities.

Photographs or audio-visual materials maybe reproduced in newspapers, brochures, poster presentations, educational and instructional manuals, on-line, website, TV media and books.  Media shared will not include any personally identifying information and will be used to illustrate the activity/activities.
