The purpose of this form is to gather information about parents who would be interested in being a chaperone for the New York trip and are ready to make the first deposit.

This form DOES NOT guarantee you a position as a chaperone and will we use service hours first and possibly a lottery system to determine the number of chaperones needed. 

By submitting the form, you agree that you have already completed an iChat form (background check form) and will adhere to the chaperone guidelines submitted by the district, directors and travel company.  iChat forms are available from the high school office.
