It is SO important that all Covid guidelines be strictly kept at Shabbos House (and beyond). We're really trying to stay open and be here for students but it takes everyone's total cooperation. If anything changes between your sign up and the event please let us know, we can change your reservation.

I confirm all of the following: *

I am not experiencing any Covid symptoms presently and will not come if I am feeling any symptoms.

I have not been exposed to a Covid-positive person that would require my current quarantine.

I will respect guidelines including mask-wearing, distancing etc at Shabbos House.

I realize risks to attending in-person events during this time, that Shabbos House is doing its best to ensure my health safety but can not guarantee it and I will not hold Shabbos House Chabad Center legally liable in the event that I do contract Covid-19.

I realize that if someone attending tests positive afterwards this form will be used for tracing purposes and if I don't follow distancing protocols it may subject me to possibly quarantine etc.
