COVID Testing Form

This form can be used to sign up for COVID testing at Fancher Creek Elementary. If your student tested positive for COVID and you would like to have them tested on DAY 5 in order to return to school on DAY 6.

My child previously tested positive for COVID and I would like to schedule an appointment to have them tested on DAY 5 in order to return to school on DAY 6.
