Please fill out application completely. In addition to this online application, you must submit the additional supporting documents to the counseling office by no later than Friday, May 3, 2019 by 4pm. Supporting documents include:
     1. Letter of recommendation from a teacher
     2. Confirmation of FAFSA or Cal Dream Act submission for eligible students or Free/reduced lunch letter.
     3. Optional for question 10; feel free to submit an Activity sheet with your supporting documents if you prefer
