Mentor Request to Begin EQ Access Process
If you feel you have an artist you have been working with that is ready to begin checking out equipment for use outside the studio, fill out this form. Your mentee will then receive an email directing them on the next training steps to initiating eq access privileges.  

All persons requesting access MUST have a mentor that approves this step and can testify to their readiness.  By completing this form you are formally placing your recommendation for the artist you identify below.
