This data intake form serves as the application for the Fred G. and Betty Jean Lee Memorial Scholarships, and the David Lee Quan Memorial Scholarship.
❖ Amount of Scholarship
Fred G. and Betty Jean Lee Memorial Scholarships - four scholarships will be awarded:
One (1) $4,000 Scholarship - The award is $1000 per year, renewable for four undergraduate years.
One (1) $1000 Scholarship - The award is $1000, issued at the freshman year level.
One (1) $1,000 Scholarship - The award is $1000, issued at the freshman year level to a student attending an accredited institution in Oregon or Washington.
One (1) $1000 Scholarship - The award is $1000, issued at the freshman year level to a student attending a trade school or community college.
David Lee Quan Memorial Scholarship:
One (1) $1,000 Scholarship – The award is $1,000 issued once at the freshman year level.
❖ Eligible Students
Be a member of the graduating class of an Oregon or Clark County (State of Washington) high school or its related equivalent.
Maintain a 3.50 or better grade point average
Be active in high school and community activities
Be a United States citizen, or hold permanent resident status
Students of all ethnicities
❖ Eligible Institutions
Eligible institutions include any two-year or four-year public or private generally accredited institution.
❖ Selection
The scholarship recipients will be selected from applicants who have completed the online application form, and have submitted all of the required documents to the Scholarship Committee as directed in these instructions by 11:59 p.m. (PT) April 01, 2024. Applications received (either online or by mail) after April 1, 2024 will not be accepted.
Fred G. and Betty Jean Lee Memorial Scholarships - The recipients for this scholarship will be selected based on the following criteria: academics, leadership in high school, and community activities. Being active in high school and community activities, especially in leadership roles is particularly important.

David Lee Quan Memorial Scholarship - One scholarship will be awarded for a student who meets the following criteria:
1. Volunteered in a healthcare or medical setting
2. Intended major is in the medical/health field

❖ Award Notification
Scholarship winners are generally notified by late May. Recipients will be required to sign and return one copy of the award notice acknowledging the terms and certifying that the scholarship will be used for educational purposes only.
❖ Disbursements
All scholarship checks will be sent directly to the financial aid office of the college attended after verification of registration as a full-time student.
❖ Terms for Renewal for the $4,000 Fred G. and Betty Jean Lee Memorial Scholarships recipient
The $4,000 Fred G. and Betty Jean Lee Memorial Scholarships is renewable for four undergraduate years at the end of each school year (June/July).
A request for this scholarship renewal must include:
1. An official transcript of school work completed that school year.
2. The student's summary report of the years' school activities
The transcripts must show that the student is maintaining normal progress as defined by the institution he/she attends.
Leaves of absence from consecutive four-year attendance must be submitted to the Scholarship Committee for approval.
Checks will be sent in four equal disbursements for each academic term/semester, after verification of registration as a full-time student.
The request for renewal along with the (1) official transcripts and (2) student's summary report must be submitted to the Scholarship Committee at: scholarships@cacaportland.com

Applicants are to fill out an online form. If difficulties are encountered with the online application form please email scholarships@cacaportland.com or leave a message at (503) 780-1925.
Application Information may also be available from most high school counseling offices throughout Oregon and SW Washington, and our website: www.cacaportland.com.
Each applicant must have their high school email or send the most recent official transcript of all high school work indicating rank in class. We do not want SAT/ACT test scores.
