Throughout the school year, junior high and high school students may be highlighted in efforts to promote Silver Lake USD 372 activities and achievements through photos, video, and interviews. For example, students may be featured on the district web site, social media, yearbook, newspaper, and video productions.


I understand that by signing this agreement, I am giving consent/declining consent for as long as the respective student is enrolled in Silver Lake Junior/Senior High School. I understand that this form becomes null and void during that time only after a parent or guardian notifies the school district in writing to withdraw consent. I also understand that failure to sign this release form within ten (10) school days from the date of distribution will constitute approval of this request.
