President's Award Nomination Form
The President Award was started by the CHPA Officers in 1993.   This award is presented by the CHPA President.  The award winner will be announced at the State Banquet and the recipient will be given a plaque.  Nominations are due by July 1.

Criteria:
The President Award usually is given for outstanding accomplishment or special service to CHPA during the past years.
