With the closure of schools all over the nation, Learn4Free seeks to provide high school and middle school students with effective tutoring and mentorship to succeed in school. Learn4Free is completely free and our tutors are volunteering their time pro bono.

Zoom is a cloud-based method of video and audio conferencing allowing our tutors and students to connect in a live setting. In an effort to do our due diligence in providing a safe learning environment, parents interested in allowing their child to use Zoom for Learn4Free video and audio conferencing with their Learn4Free tutors, must acknowledge the privacy policies of Zoom and complete the consent form below authorizing their child to participate. Your signature of this contract will signify your understanding of these terms.

Please complete the form below ONLY if you are granting permission for your child to participate and agree to the policies and provisions outlined below. Completion of this form does not guarantee placement in Learn4Free.

I authorize my child to participate in Learn4Free sponsored live zoom meetings for the purpose of instructional content and mentorship by Learn4Free Tutors. I understand that the parent is responsible for the home learning environment. This includes ensuring that my child is properly supervised and that the background environment is appropriate for a classroom setting. I understand that all live Zoom meetings are to be treated as a classroom setting. Students are expected to be respectful to the tutor, remain engaged in learning, and be cooperative at all times. I understand that my child must be fully and appropriately clothed in order to participate in a live video zoom meeting. Students not that do not follow this policy cannot participate in Learn4Free Tutoring. I understand that participation at Learn4Free Zoom meetings is necessary in order to maintain enrollment, all meetings will be recorded and privately uploaded to the appropriate platform for quality assurance purposes. I understand that it is my responsibility as a parent to supervise my child and make sure he or she is cooperative with our tutors and willing to actively learn. I understand that I or my child will need to make prior arrangements with Learn4Free tutors at least 1 day in advance if an absence is necessary. Students who are absent for more than 2 sessions in 1 semester will be withdrawn from Learn4Free as students and are unable to re-enroll.
