Welcome to the Animal Progress canine behaviour questionnaire.

Information provided on this questionnaire is confidential, however I would like to be able to discuss the treatment plan with your veterinarian for better long-term care of your pet.  

Video of your pet's behaviour can be highly beneficial but please do NOT take video  if it is likely to encourage any aggression or if taking the video limits your ability to intervene to stop an aggression incident.

This questionnaire is long, so do not attempt to do it on your phone. Use a computer. Set aside a good chunk of time to devote to it. Behaviour problems are complex and are influenced by many factors. The questionnaire helps guide me to a diagnosis. The more detail I have, the better I can understand your dog's problem.

To fill out the questionnaire, please read the description of each section under the section heading. There may be several sections that are not relevant to your dog's problem. I have occasionally provided a yes/no question so that you can skip sections that are not relevant to your dog. Some sections are only 1-2 questions, some are longer.
