Hello A3D3 members. In an effort to ensure that all institute members have the ability to report things anonymously to the institute leadership, we've added an "Incident Report Form" to the Website. This form will only be sent to the A3D3 Equity & Career Committee co-chairs (Javier Duarte and Mia Liu).This form is for those individuals who wish to remain anonymous or want to report something, but don't exactly know what they should do.
This form is a proactive way to ensure that lines of communication are open and that there exists a way for the institute members to report incidents and maintain anonymity. Should you have any questions please feel free to contact us at jduarte@ucsd.edu and liu3173@purdue.edu.
Note, if you want to remain anonymous, we will make every effort to do so. You can still put your name and email for the co-chairs to follow up with you, and we will keep it confidential.
