Thank you for considering Cedar Hill ISD for your event.  You will need to fill out this online form completely to begin the rental process.  Once we have reviewed your request, we will contact you to continue the process.

All requests for facility usage must be received no less than 20 and no more than 90 days prior to the requested date(s).

ALL ORGANIZATIONS MUST PROVIDE AN ACORD CERTIFICATE OF LIABILITY INSURANCE, LISTING CEDAR HILL ISD AS THE CERTIFICATE HOLDER AND ADDITIONAL INSURED, BASED ON THE REQUIREMENTS OUTLINED IN SECTION 2 OF THE GKD REGULATIONS (EXHIBIT A):  https://www.chisd.net/site/handlers/filedownload.ashx?moduleinstanceid=15987&dataid=22362&FileName=GKD%20Exhibit%20A.pdf

Your Acord Certificate of Liability Insurance must include coverage for the following:

1.  Commercial General Liability (min. $1,000,000 per occurrence)
2.  Automobile Liability (min. $500,000 per occurrence - if organization is transporting anything onto school property)
3.  Workers Compensation (min. $100,000 per occurrence - if organization has hired staff or is contracting services)
