This is a legal document to be completed with full government name and DOB. I am at least 18 years of age (OR have clearance by a legal guardian 21+) and hereby agree to: release and forever discharge Vertical Joe’s Dance and Fitness Studio, its agents, servants, employees, independent contractors, officers, directors, trustees, owners and all other persons or entities on its behalf from any and all lawsuits, claims, actions, damages, liability, cost, expenses or fees that are in any way connected to my participation in this event.  I acknowledge and understand that attending, participating or assisting someone to participate in the events will involve strenuous physical exertion, require a significant degree of skill and could be physically hazardous or dangerous.  I hereby represent and properly warrant that I have the physical ability to participate properly and safely.   I am fully aware of and voluntarily accept and assume risks, hazards and dangers, including any and all risk of loss, damage, property damage, injury or death as a result of participating in events. Any damage to Vertical Joe’s equipment or property will be paid at my expense. I am not pregnant.  I am not exercising with a preexisting condition that has not been cleared by my doctor.
