In line with our school's SOPs, visitors must make an appointment prior to collecting their certificates. This form is strictly for booking of appointment on selected days for collection of IGCSE/O-Level/A-Level Official Certificates and Statement of Results only.

This form must be submitted before collection of Certificates and Statement of Results is granted. Walk in-s without any appointment will not be entertained.

Upon filling up this form and submitting it, you are agreeing to these terms and conditions.

1. All information must be completed and information provided must be true and accurate.

3. Representatives collecting the SOR or Official Certificates on behalf of the candidates will need to 𝗯𝗿𝗶𝗻𝗴 𝗮𝗹𝗼𝗻𝗴 𝗮 𝗰𝗼𝗽𝘆 𝗼𝗳 𝘁𝗵𝗲 𝗰𝗮𝗻𝗱𝗶𝗱𝗮𝘁𝗲'𝘀 𝗜𝗖 𝗼𝗿 𝗣𝗮𝘀𝘀𝗽𝗼𝗿𝘁 𝗮𝘀 𝘄𝗲𝗹𝗹 𝗮𝘀 𝘁𝗵𝗲 𝗰𝗼𝗺𝗽𝗹𝗲𝘁𝗲𝗱 𝗰𝗼𝗽𝘆 𝗼𝗳 𝘁𝗵𝗲 𝗔𝘂𝘁𝗵𝗼𝗿𝗶𝘀𝗮𝘁𝗶𝗼𝗻 𝗹𝗲𝘁𝘁𝗲𝗿.
