2024 B-Smart Daycare & OSC Registration Form
Welcome to L & J Day Care Centre Ltd. Operating as B-Smart Learning Center inc. and thank you for choosing us to provide your child the highest quality of care and early learning experience.

Criteria:
Your child must be at least 6 months at the time of registration and not older than 11 years.

Payments:
Registration fee ($150) 
Space Deposit fee ($250) refundable if parents give 4 weeks notice before last day of attendance
Hi mama yearly technology fee ($15)
Arabic Program yearly fee ($100) 

Please note that your children must attend FULL TIME (a min of 100 hours for Daycare & a min of 50 hours for OSC- Out of School Care) to benefit from the full approved subsidy. If your child(ren) makes less than the above mentioned hours, the daycare has to charge a higher parent portion to make up for the missing hours unattended as the government does not cover these missing hours.

Fees & Subsidy
Monthly Fees: The below fees are before Affordability Grant deduction and Subsidy deduction. For example: If your child attends the Pre-K room, their monthly fee is $886-$626. If parents are approved for full subsidy, please deduct an additional ($260) the monthly parent portion is ZERO FOR BASIC FEE (Meal fee not included in the basic fee package)

Subsidy & Affordability Grant
Subsidy: Eligible lower- to middle-income families can apply for child care subsidies if they have a child enrolled in:
licensed daycare centers and licensed out-of-school care centers

Affordability Grant:  All parents are approved for a reduction of fees according to the child(ren) age group. Please refer to the table below:

Emergency Medical Care If my child requires emergency medical attention
AUTHORIZATION AND CONSENT OF PARENT(S) OR LEGAL GUARDIAN (S)I do hereby state that I have legal custody of the aforementioned Minor. I grant my authorization and consent for ___ L & J Day Care Centre Ltd O/A B-Smart Learning Center Inc. (hereafter "Designated Adult”) _ to administer general first aid treatment for any minor injuries or illnesses experienced by the Minor. If the injury or illness is life threatening or in need of emergency treatment, I authorize the Designated Adult to summon any and all professional emergency personnel to attend, transport, and treat the minor and to issue consent for any X-ray, anesthetic, blood transfusion, medication, or other medical diagnosis, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of, any licensed physician, surgeon, dentist, hospital, or other medical professional. I agree to assume financial responsibility for all expenses of such care including ambulance fees. It is understood that this authorization is given in advance of any such medical treatment and is given to provide authority and power on the part of the Designated Adult.

Neighborhood Walks Permission 
I understand that community excursions or walks in the immediate neighborhood are part of the program at B-Smart Daycare & OSC and I hereby give consent for my child to participate in these activities.

 Photographs & Displays of Children’s Work

I hereby give my consent to B-Smart Daycare & OSC to take photographs of my child. I also give consent to the B-Smart Daycare & OSC to display my child’s work or projects, and photographs of my child in the daycare/OSC rooms and on the B-Smart Daycare & OSC social media ( Facebook and Instagram page).
