I DO wish to donate my artwork to be sold.  I understand that I will establish the retail value of my artwork.  I also understand that I will receive a letter of donation with that retail value for tax purposes as a Charitable Donation.  I further understand that the proceeds of the sale will be donated to the HELMS Foundation and that I will NOT get my artwork back.

Releasing my rights includes, but is not limited to:  The Ryan Giambattista HELMS Foundation for use in public display, and/or photographing my artwork for the 5th Annual Helms Foundation Fundraiser, as well as the HELMS Foundation website.
