Welcome! If you are on this page, its because a friend recommended you get into this, you've always wanted to try but didn't know where to start, or Ms. Meikle has directly sent you this form to fill out. 
Thank you for your time! Congrats on figure modeling. Some questions below just help Ms. Meikle get an understanding of your background and what we are working with. 

Ms. Meikle Currently hosts at Free Range Brewery in NoDa on the 1st and 3rd Sundays of the month from 11am - 1pm. Payment is $20 per hour, and we get tips from the artists. Cash tips are split between the host and model (75%/25%). Digital tips go straight to each individual.

Yes, we welcome people who are 'getting back into it'. Would this be your first time? Have you ever tired other physically demanding art forms?

Are there any medical adjustments or requirements I should know about? 
example but not limited to: Peanut allergies, problems with fainting, limited appendages, previous car accidents or serious injuries. 
Ms. Meikle currently only hosts at Free Range Brewery located in NoDa. The class is located in the back with a Large Garage window that faces the public. People would have to Really Try to 'observe' by pressing their face against the glass. We now have a curtain to cover the window, and we position the model off of the stage and in a radial composition (artists sit in a circle around the model) that way everyone has a great angle. 
On rare occasion's people walk through to get to the bar, or an employee needs to grab something. Everything is safe, nothing has happened, or will. But this environment is not for shy people.
