JCAN 2024 RECOGNITION AWARDS
JCAN, is a non-profit charitable  organization serving Black communities and other underserved populations impacted by the social determinants of health.

Nomination Categories:

Inspiring Healthcare Leader Award:
Honours individuals who exemplify exceptional dedication, compassion, and innovation in the healthcare field. Nominees should go above and beyond their routine duties, making a lasting impact on patients, colleagues, and the community through their leadership and innovation

Legacy Award :
Recognizes individuals who have retired from the healthcare field but continue to contribution to the health and well-being of Black communities, leaving a lasting legacy.   

Senior Care Luminary Award: 
Advocate for seniors’ wellness. Nominees should demonstrate exceptional leadership in improving the health and well-being of the elderly in our communities.

Community Service Award: 
Recognizing an individual or agency making a significant impact on the health and wellness of Black communities.  This award celebrates outstanding community service contributions.
