All members under the age of 18 should completed this form only including:
Youth Members - Under 18s that play for senior teams
Junior Members - Playing Beavers or Badgers or attending Sunday morning Trainings

All members are required to complete a membership form each season. Information you provide allows us to keep you informed about training, fixtures and club social events.

Membership renewals are due on 1st September with payment arrangements confirmed by 15th September.
Failure to pay on time could result in you being excluded from team selection.

You also need to choose your category of membership and tell us how you want to pay.

All details will be kept in a secure database with access restricted to authorised club officers only.

PLEASE NOTE: IF YOU WANT TO PAY BY INSTALMENTS THE FIRST PAYMENT IS DUE BY 15th SEPTEMBER .

SECTION 2: MEMBERSHIP & PAYMENT METHOD

We have 2 membership categories and you choose the one that suits you best. You can pay for the whole year in full or in two instalments (one in September and another in January).

Youth Members (£100): Under 18s playing senior hockey - one instalment of £100 or two instalments of £50

Junior Members (£80) - Junior Annual Members playing Badgers or Beavers or training Sunday Mornings - one instalment of £80 or two instalments of £40

Junior Members (Pay as you go £5 a session) - Junior Annual Members playing Badgers or Beavers or training Sunday Mornings.
