Pursuant to Section 9 of the Tri-City Operating Rules, this form should be completed to request usage of our name or logo onto items such as apparel, banners or displays.

In addition to completion of this form a sample or mock-up of the item MUST be sent via email to merchandise@tricityeagles.com.
