Take-Home Rapid Antigen Request Form

Organizers of Dartmouth-sponsored in-person events who would like to make take-home antigen tests available to event attendees (students, faculty, staff, and visitors) can fill out this form to make a bulk purchase. Each test costs $4.63.

Masks can be ordered in bulk directly from Central Stores by calling 603-646-3289 or emailing Central.Stores@dartmouth.edu.
