The 2023 Sullivan Annual Awards Banquet will take place on April 13, 2024 at 7T Farms in Sullivan. You may use this nomination form to submit nominations no later than January 13, 2024. Please state at least one reason you feel the person/business/organization deserves to be nominated. Voting will be based on the information collected and presented on each nominee. The Banquet Committee reserves the right to re-categorize entries into proper classification. Winners will not be announced prior to the Banquet.

MAN OF THE YEAR AWARD - This award is presented to a gentleman who has unselfishly demonstrated outstanding service to and for our community and mankind during 2023.

WOMAN OF THE YEAR AWARD - This award is presented to a woman who has unselfishly demonstrated outstanding service to and for our community and mankind during 2023.

OUTSTANDING YOUNG MAN AWARD - This award is presented to a young man of the area, is a senior in high school and whom has dedicated himself to the betterment of our community.

OUTSTANDING YOUNG WOMAN AWARD - This award is presented to a young lady of the area, who is a senior in high school and whom has dedicated herself to the betterment of our community.

FUTURE YOUNG LEADER AWARD - This award is presented to an individual between the ages of 8 and 16 of the area, who has displayed leadership, dedication to the community and assisted various businesses/organizations throughout the year for the betterment of our community.

NEW BUSINESS OF THE YEAR AWARD – This award is presented to any business owner/manager that has shown a true desire to provide employment, services, and/or betterment for the area during 2023. This business should be established for between 6 months and 2 years.

BUSINESS OF THE YEAR AWARD - This award is presented to any business owner/manager who is a current Chamber member and has shown a true desire to provide employment, services, and/or betterment for the area during 2023. This business should be established for 3 or more years.

FIRST RESPONDER HERO AWARD - This award is presented to a qualifying first responder whom displays heroism, bravery, community service, and dedication beyond the call of duty. The first responder must be one that is an active member of an emergency service organization; which includes police departments, sheriff departments, fire departments, ambulance services, and other emergency response/rescue services within the Sullivan area.

EDUCATOR AWARD - This award is presented to a teacher, administrator or counselor that has exceeded expectations and dedicated themselves to the enlightenment and development of our area youth.

PUBLIC SERVICE AWARD - This award is presented to an employee/volunteer/organization of a tax or public supported entity for outstanding service above and beyond expectations (i.e. city, organization, church).

BEAUTIFICATION AWARD - This award is presented to a person or business who has exerted the extra effort to make Sullivan a beautiful place to live, work and visit.
