Be Advised - Boarding Requirements & Policies & Procedures: 1.) If pet medications require a pill pocket, please provide them, or we will provide them for an additional cost.
2.) If your pet(s) gets anxiety during storms, we can provide medication to calm them for an additional cost.
3.) WSVC proudly provides Blue Buffalo for pets that are boarding. If your pet is on special diet and you are providing the food during boarding; please bring in individual portioned bags for each day (with feeding instructions).
4.) All pets must be current on all required vaccinations listed at the top of this form.
5.) If your pet(s) have not been examined at WSVC before we are required to do an exam to establish a client patient relationship.
6.) If your pet(s) have loose stools, diarrhea, or vomiting during their stay with us, we will examine your pet and give medications that are necessary.
7.) All pets must be free of external and internal parasites (ie: ticks, fleas, etc.) or they will be treated at the owners expense.
8.) Williams-Sebby Veterinary Clinic has my permission to do WHATEVER NECESSARY should an emergency arise with my pet.
9.) Your pets may be picked up between 7:30am - 5:45pm Monday - Friday and before 11:45am on Saturday. NO EXCEPTIONS.
***IF YOU PICK UP AFTER 10:00AM O ANY BUSINESS DAY, YOU WILL BE CHARGED FOR ANOTHER DAY OF BOARDING.
10.) Beds, towels, toys, etc. must have last name on them. This allows us to identify your items if it becomes necessary to wash them. IF ITEMS ARE NOT LABLED WE CAN NOT BE HELD ACCOUNTABLE FOR RETURNING THEM TO YOU. PLEASE MARK ALL YOUR PETS BELONGGINS WITH YOUR LAST NAME!
My signature on this document serves as my consent to all WSVC policies and procedures, regarding the boarding of my animal. I understand that buy agreeing to these conditions; I am responsible for any additional fees, charges, or costs associated with caring for my animal while boarding with WSVC.
