خصصت هذه الاستمارة للراغبين بتقديم الدعم لبرنامج تطوير جودة التعليم في مؤسسة فيصل الحسيني ولبرنامجها لدعم المشاريع الصغيرة والطارئة لمؤسسات المدينة الشبابية والثقافية والصحية وغيرها، وتأتي معظم أسئلتا بناء على الأسئلة التي تطرحها البنوك لدى وصول التبرعات، إضافة إلى أسئلة خاصة بسبل التواصل معكم. شاكرين لكم اهتمامكم وتبرعكم الكريم.  This survey aims at collecting information from those interested in supporting Palestinian schools in Jerusalem, through our school development program, and those who are interested in supporting  youth, cultural, health and other institutions in Jerusalem through our Small and Emergency support Program . Most of the questions in the survey will help us in answering the questions that will be asked by banks concerning the donation, in addition to questions concerning your contact information.
