This Bus Service Request Form is required for parents & caregivers requesting Bus Service for their eligible Moose Factory Ministik School Students from August, 2022 - June 2023.

THERE ARE BIG CHALLENGES TRYING TO BUS SO MANY CHILDREN TO SCHOOL EACH DAY. IT WILL BE A HUGE SUPPORT TO MCEA AND THE SCHOOL IF PARENTS & CAREGIVERS CAN PROVIDE THEIR OWN TRANSPORTATION AT THIS TIME.

*Bus Services are operated by the MFIDSAB for Kindergarten to Grade 2 students residing off-reserve (south of GG's).

*Bus Services are operated by the MCEA for Kindergarten to Grade 6 students residing on-reserve (north of GG's).

This Bus Service Request is to be completed by the custodial parent, guardian or caregiver of student(s) registered with Moose Factory Ministik School who are eligible and are requesting bus service.

This number will be used in emergency situations or for follow up communication.

Please list the first and last names of Ministik Students in your home who require bus services. *
