2022-2023 Quaker Marching Band Registration Form
This Form is due Thursday, June 3, 2022 - The band fee for the next season is $227 which includes a QMB Show T-Shirt, 3 band camp dinners, chicken BBQ dinner (for member), snacks/beverages/games at Band Camp, family picnic, pizza dinners before home football games, show props, coach bus transportation and one night accommodation for our overnight trip to Syracuse for the championship performance at the Carrier Dome and the Annual QMB Banquet.)

Show theme will be announced at the Parent and Member June Meeting on Wed. June 8th

QMB Leadership: If you are going to be in grades 10-12 beginning in the Fall of 2022 you may apply for a leadership position as Section Leader or Drum Major. (There will be a separate form for this process.) *
I want to be in The Quaker Marching Band for the 2022-2023 school year.  My parents and I have read and understand guidelines on the initial information flyer. I understand that a QMB fee of $227 ($177 for each additional sibling) is due by June 8, 2022 (the Parent/Member Meeting) or feel free to mail it in to PO Box 187, O.P., NY 14127 *
I understand that Band Camp Week is not optional and that I am doing everything possible to attend ALL days of the camp at OPHS from August 22-26, 2022. *
