Please fill this form in separately for each child in the school.
If your child is successful on getting into a club they will receive a letter confirming their place. Filling this form in does not guarantee a place in a club.
Children must be collected promptly at 4.15 (3.00 on Fridays) or will lose their place in the club.
· In the event of any behaviour that does not meet our high standards, pupils will be given one warning - any further incidents will lead to your child not returning to club.
· Appropriate sports and footwear must be worn at sports clubs, uniform for all others.
· Children may bring a drink.
· If children do not turn up to club, they will lose their place unless it is a genuine medical reason as there are many children on waiting lists.
· Siblings, who are not in a club, MUST be collected at 3.15/3.20 (2.00 on a Friday) or your child will lose their place in club
· Clubs start the week beginning 9th January and run until Easter.
