Weekly Meal Pick-up Form for any child 18 and under who is not receiving an onsite meal at Nashville Schools.

This Google document will be filled out to order a week worth of breakfast and lunch monthly. Please select the dates you wish to pick up meals in the month. If you are not going to be able to pick up the meals in a week after you sign up, please call 870-845-3425 and let Julie Smith know so we do not prepare the meals.
A drive through pick up will be Wednesday from 1:00-2:00 at Nashville High School outside the Cafeteria employee entrance. Please remain in your cars.
All meals will be free.
Parents will be allowed to pick up meals for the student.
Student need not be present during pick up.
All meals served must be accounted for by school staff, Students will be recorded in the POS system. Children who are not enrolled in Nashville Schools will be counted by Name.
