2021-2022 NCRHA Team Commitment Form
All teams need to submit their Team Commitment Form for the 2021-2022 season.

This form needs only be completed once per club.  Responses are due by September 10, 2021 at 11:00PM Eastern.

Team Commitments must be completed every season in which the club intends to participate.

Please read the information below, indicating you understand the requirements at the bottom:

1. This form is a commitment for the 2021-2022 season.  

2. Unlike previous seasons, this is not a Divisional Application Form. The intent is to get teams registered for the season, and then have teams work with their conference directors and the NCRHA to begin the season.
