As part of the ministry of St Mark’s Church, we aim to offer a varied programme of activities, learning and worship for young people. As part of our commitment to safeguarding children, we maintain up-to-date registration forms for each young person in our care.  Please complete the following form and return it to the group leader or one of the clergy.  Completed forms will be stored securely and will remain confidential within the group of leaders.
