a) I acknowledge that I am aware of the various activities that will be conducted at Silver Talkies Club, club for the 55+ by Silver Talkies. I warrant that the above-named member enrolled is mentally and physically capable to participate in these activities/ program.
b) In the event of medical emergency involving me, I consent to Silver Talkies seeking all necessary medical assistance and respond to that emergency. I agree to indemnify all the organizations against any cost they might incur in seeking such medical assistance.
c) I undertake and agree to hold Silver Talkies and its officials and staff indemnified against all such claims that may arise out of any loss or injury to the property or person (including injury resulting in death) which I may suffer while or in consequence of participation in any activity. All expenses that may be incurred in the treatment of such injuries will be borne by me.
d) I also undertake and declare that I or any of my representatives will not claim any compensation from Silver Talkies on account of any such injury or loss as described in paragraph (c). l hereby promise to indemnify and keep indemnified and harmless Silver Talkies, the organization, their employees/officials, from every type of loss(es) or damage(s) which may arise during the said activities for the entire tenure of the Silver Talkies Club.
e) Silver Talkies’ management reserves the right to remove me from the program/ club where my behaviour is, in the opinion of Silver Talkies Club team, seriously disruptive or inappropriate. Any refunds would be as per Silver Talkies Club policies.
f) I acknowledge that occasionally I may be photographed for publicity purposes and I will inform the Silver Talkies staff if l do not want me to be photographed. Such photographs may be used in Silver Talkies collateral and for publishing in social media of Silver Talkies and Silver Talkies Club. I will inform the Silver Talkies staff in advance if I do not want my photographs to be used for such purposes. I also have the right to request all concerned parties to not use my photograph in the future and remove it where possible.
g) I give permission to the Silver Talkies management to share my contact details with the sponsors to receive communications from them
h) I agree that all Silver Talkies related communications be sent to me via email/ SMS/ Whatsapp as required.
i) I am competent to make the above said declaration.
