Thank you for your interest in the Mathematics Mentoring Academy for Teachers of Exceptional Students (MathMATES) for the 2022-2023 school year! The goal of MathMATES is to develop processes that support mathematics-centered collaboration between Mathematics Teachers and Special Education Teachers focused on meeting the diverse needs of exceptional learners.

By completing this form, you are indicating that a mathematics co-teaching team from your school or district is interested in applying to the MathMATES project. Each team must be comprised of AT LEAST ONE general education Mathematics Teacher and AT LEAST ONE Special Education Inclusion Teacher who work with 6th, 7th, and/or 8th-grade students with exceptional needs. All team members must have shared responsibility for the mathematics instructional support of exceptional students. These partnerships must be maintained throughout the 2022-2023 school year.

To complete this form, you will need the names and email addresses of all teachers interested in applying to be part of your MathMATES team. You will also need the name and email address of the administrator (usually a principal) that is responsible for guaranteeing the provision of any administrative support needed to complete this project. Examples of such administrative support include providing time off to attend Fall and Spring follow-ups, allocating common planning time for the team, etc.

Individual teachers may only participate as part of one team. Please understand that space is limited, thus selected participants must agree to attend all scheduled events. After submitting this interest form, each team member will receive an email inviting them to complete an individual application. All team members must complete their individual applications by May 1, 2022, to be considered.
