Unit COVID-19 Pre-Screening Checklist
Every member and guest attending the in-person training operation must, prior to entering the facility or training area, read and acknowledge the following COVID 19 PRE-SCREENING questions and submit the form BEFORE arriving at the training facility/area for the date of the activity in question (one form submission per person, per date).

THIS FORM CANNOT BE USED FOR MORE THAN ONE INDIVIDUAL (EVEN WITHIN THE SAME FAMILY).

No person shall be permitted on-site until they have completed this form. No person shall be permitted on-site if they have answered ‘Yes’ to any of the following questions.

If there are any concerns (answering ANY of the following screening questions, 'Yes') shall result in that individual not being permitted to participate in ANY in-person training and/or entering the training facility/area. They shall also be required to meet ALL local public health and Canadian Cadet Organization prerequisite protocols before being allowed to return to any in-person training and/or attendance.
