Computer Information (Please fill out only 1 form per student).
By filling out this form you begin the process so we can get your username and password set up which can take 3-5 days.

Upon receipt of the computer, each student will confirm the equipment’s condition, at which point the student/parent/guardian will assume custody and full responsibility of the computer, including battery, and power supply. Students/parents/guardians are expected to abide by the terms of the Laptop Handbook at all times.

Upon registration approval, you may pickup a laptop at our Clearfield Office or we will contact you to make other arrangements depending on where you live.
Non-full time students and SEOP students may be charged $40/semester for computer rentals.
