The aim of this survey is to find out whether people are aware of the various health and wellbeing services available to them in their community or online. The information you provide will be used to inform our approach to communications and engagement and is the start of a process that will include in-person focus groups and conversations with people across the community and online.

 We would like to know if you are aware of the services listed below, whether you have accessed them and if they have met your needs.  We ask some questions at the end about gender, age and post code. These will help us to identify whether there are any differences in responses among these groups. We understand if you would prefer not to answer these questions.
