**WE WORK ON A REFERRAL ONLY BASIS**

** This form MUST ONLY BE COMPLETED in a professional capacity and we are unable to accept private referrals. **

You will need to ask an organisation or service to refer you to us. A proffesional must complete this form. This can be your GP, school, food bank, health visitor, social worker, support worker, family nurse, midwife, children and family center, citizens advice or similar organisation. It is really important for us to continue to provide these services that you get a referral. 

Referral's will be completed as soon as possible, prioritising babies due dates that are sooner or urgent referrals.

PLEASE NOTE:

**Requests MUST BE COLLECTED within 7 days of us notifying you that they are ready.  We do not have the capacity to deliver or store bundles ** 

**We cannot provide food parcels. We may be able to provide a small amount of baby food, and Stage 1 formula, if requested. If you are in need of support with providing food for yourself or your family, please contact your local foodbank. On our website we have listed some other local organisations who can help with food and other community support.**

**Our service is available to anyone living in Sussex. If you are outside of our area, please search for your local baby bank, or email us for help at the address above.
