Cornish's response to the COVID-19 pandemic is designed to sustain our important mission of education while maintaining the health and safety of our faculty, students and staff. Washington State has updated guidance requiring employers to provide accommodations to high-risk employees. Please review this updated information:
https://coronavirus.wa.gov/information-for/workers/high-risk-worker-proclamationCornish employees who request a temporary modification to their work assignment based on high-risk concerns may be eligible for Temporary Remote Work, if appropriate. Other modifications may include options for physical distancing (including staggering shifts), alternative work locations, reassignment, modified or flexible schedules, and/or the use of leave.
Cornish may provide temporary workplace modifications due to COVID-19 to qualified employees. In general, it is the employee’s responsibility to inform the College that they need a COVID-19 related modification. Cornish is not required to provide modifications if they are not aware of the employee’s need and desire for one. Temporary modifications are determined, identified and implemented in a collaborative process among the employee, supervisor and Human Resources. All medical documentation and information should be shared solely with HR, not the supervisor.
Contents of this request are confidential and will only be shared as needed with the appropriate personnel to consider the implementation of a temporary workplace modification. This form will not be placed in your employment record file. All medical documentation will be kept confidential.
