This Registration is for the Pushpa Medley Workshop from May 17th to June 7th

Tuesday, May 17th - Oo Anatava Part I
Tuesday, May 24th- Oo Anatava Part II
Tuesday, May 31st- Saami Part I
Tuesday, June 7th- Saami Part II

Total 6 hours - Fees - $80 for the full Workshop
$25 Drop-in class
Fees must be paid on the first class or prior to the first class through online transfers.

I understand and accept that I am solely responsible for as self/guardian for the aforementioned student and any or all injuries/ill-health that may be incurred during the scheduled period of time in which the student is enrolled. Spinning Canvas, LLC and agents associated with the company, including but not limited to, the dance studio and related parties, the choreographer and related parties, and instructors may not be held legally liable for an injury/sickness that may occur to the student during the time period that the student is partaking in the above said activity.
Photos and videos may be taken during classes or any performances and will be solely used for promotional and marketing purposes of Spinning Canvas, LLC.
